Acne
Acne is a skin condition that affects adults just as it affects teens. If it gets too severe, it can leave lasting scars that may last a lifetime. Our specialist team of Dermatologists and skincare experts at Atlantic Dermatology and laser, treat teens and adults living in Atlantic seaboard, Cape Town, as well as the surrounding areas, dealing with acne to help control it and diminish the appearance of breakouts.

What Is Acne?

Pimples (acne) are small swollen red bumps that result from excess oil that causes pore openings to become clogged. The openings become red, swollen, inflamed and ultimately infected. At this point, a whitehead (white pustule) develops on the top of the pimples becoming increasingly larger and more obvious. Untreated acne can get worse and, for some, leave scars as it heals.

What are Acne Treatments?

There are many acne treatments on the market that target breakouts by penetrating the pores in the skin and removing excess oil, dirt, and bacteria, decreasing the chances of reoccurring pimples. Most acne treatments prescribed by Dermatologists are topical solutions and ointments that effectively treat this common issue. For those with more severe problems, oral medications and laser therapy are used to achieve successful results.
Acne treatment controls and clears up breakouts as well as provides benefits, such as:
• Inhibits acne scarring
• Restores a smooth complexion
• Little to no downtime following treatments
• Lessens the severity of future breakouts
How Do I Prepare for Acne Treatment?
Topical and oral medications are typically prescribed then applied or taken at home according to the directions. For laser therapy, it is important to avoid direct sun exposure prior to your treatment as well as stop taking any blood-thinning medications or supplements that may hinder the healing process. During a consultation for laser treatments, we provide you with the necessary serums and moisturizers to prep the skin. In-office acne treatments take place when laser treatments and chemical peels are used. This involves using laser energy on the affected areas to remove damaged skin. These laser treatments also stimulate collagen production, which in turn boosts new skin growth. As for topical and oral treatments, they are prescribed and typically handled at home. It is important to follow the directions for taking or applying the acne treatment. Many prescription treatments have successful results with acne, especially when combined with the appropriate skin care products for acne prone skin.

When Will I See the Results?

Depending on the selected acne treatment, the results take some time as the current breakouts must clear up and future breakouts become less severe. As for laser treatments, the treated skin must heal, and new skin must take over. Following acne treatment with laser treatments, the treated area may have slight redness that takes a few days to dissipate. There may also be some slight flaking as the redness diminishes. Once these issues resolve themselves, the skin starts to look smooth and clear.

Dark Circles
Carboxytherapy can dramatically improve the appearance of dark under-eye circles. Although sometimes caused by darkened pigment, or a hollow depression below the lower eyelids [tear trough deformity], the majority of dark under-eye circles are caused by poor circulation beneath the lower eyelids [vascular pooling]. During the first study in the English literature using carboxytherapy for rejuvenating the under- eye region. they found that by injecting a small amount of carbon dioxide gas just beneath the skin of the lower eyelid, circulation was increased and dark under-eye circles were markedly improved. The treatment takes only five minutes, and is virtually painless and risk-free. A series of 2-6 treatments spaced one week apart is all that is required to achieve a great result.

From stress to those who are overworked, dark circles around the eyes is something that plagues a lot of people. There are lots of ways adults and children can develop these dark circles under their eyes. Yes, some people have dark circles under their eyes because of lack of sleep or too much alcohol. But those aren’t the only reasons. Let’s talk about other possible reasons why you have dark circles under your eyes and what you can do about it.

First, let’s understand how dark circles appear. We have tiny blood vessels, which are like a web under the skin. But these capillaries are so fine that the red blood cells queue up to pass through; in the process some of them leak in the surrounding area. Enzymes are produced during the cleaning up session. The breaking down of these red blood cells leaves them black and blue. The reason why this is so visible is that the skin around the eyes is the thinnest.

What Is Causing the Dark Circles Under My Eyes?

  • Aging – The skin under the eyes is thin and delicate to begin with. As we grow older, skin around the eyes becomes thinner making blood vessels more prominent, causing dark circles.
  • Genetics – Hereditary and genetics can also play a big role in the development and dominance of dark circles around the eyes.
  • Nutritional deficiency – Dark circle around the eyes can be due to poor nutrition. A healthy and nutritious diet filled with vitamins like A, C, K, E and nutrients can help to get rid of dark circles.
  • Sleep deprivation and tiredness – A lack of sleep or excessive tiredness results in pale skin, making blood under the skin become more visible and appear more blue or darker.
  • Smoking and drinking – Late night parties, smoking and drinking can play havoc with your skin and result in dark eye circles. Dark eye circles could be a sign of loss of water from the body (dehydration) due to excessive drinking and intake of caffeinated drinks.
  • Sun exposure – Increased exposure to the sun can draw pigmentation of the skin’s surface and create dark circles. Long sun exposure produces more melanin around eyes than usual, giving them a darker color.
  • Hormonal changes – In women, the skin undergoes lots of changes during the phase of pregnancy and menstruation, causing darkening under the eyes.
  • Allergies – Any condition that causes the eyes to itch may contribute to darker circles due to rubbing or scratching the skin around them. Apart from that, some food allergies can also cause the area under the eyes to appear darker. In fact, dark circles in children often indicate allergy problems. Research also suggests that dark circles are linked to anemia, and liver diseases.
  • Dark circles and anemia – Iron deficiency is one of the most common causes of inexplicable dark circles in many cases, which can be treated by making simple changes in your diet. Low iron levels is the most common form of anemia, and results in poor oxygenation in body tissues due to low supply of oxygenated blood. Thus, always take a balanced diet rich in green leafy vegetable, dairy products and all types of fruits to keep your body healthy.
  • Dark circles and dehydration – Dehydration is one of the most common reasons for dark circles under the eyes. The reason is the close proximity to the skin underneath the eye in relation to the underlying bone. When the body does not have a proper amount of water, the symptoms are often evident in this specific area. Thus it is advisable to have at least 8-10 glasses of water and include fresh fruit daily. It is also advisable to restrict the intake of caffeinated drinks such as tea, coffee, alcohol and other caffeinated drinks as it aggravates dehydration.
  • Dark circles and liver disease – Dark circles also indicate liver dysfunction due to various liver ailments. An example of such a liver ailment is hepatitis.

Laser Hair Removal
The warm weather does wonders for our souls, but we could certainly do without the extra time it takes to shave, wax off every exposed hair on our bodies. Before you reach for that razor, make sure your preferred hair-removal technique is also kind to your skin. There are a few Hair-Removal Techniques, However Laser hair removal is rated the quickest and most effective method, and in the long- term, the most cost effective as well.

Shaving

The easiest, and one of the cheapest, methods we rely on is also one that can leave our skin bumpy, stubbly and even bloody. Because the razor cuts the hair at an angle, it has a tendency to regrow inward (causing ingrown hairs), particularly if hair is curly, the result is bumpy and inflamed, infected skin which will not resolve until the ingrown hair is removed. Possible complications from shaving can include scarring, breaking fine capillaries when removing the hair and permanent dark marks (hyperpigmentation) after healing has completed. One possible benefit-The razor itself does have a moderate exfoliating effect, removing dead surface skin cells as you shave.

Hair-removal creams

The main risk with these products is chemical burns because they are strongly alkaline. A burn is more likely if you leave the product on for longer than directed on the packaging. Some hair types are not removed effectively within the directed timeframe for use therefore patients will leave them on longer. By the time the hair is dissolved the skin is also burnt. Know your skin — if it’s sensitive, it may be best to stay away from these creams. And, if you do use them, make sure you’re removing them thoroughly or you risk developing a “slow burn.”

Electrolysis

Electrolysis has pretty much been replaced by lasers, which are faster and more efficacious. If you’re still devoted to this method, be aware that risks include “scarring, infection and hyperpigmentation.”

Threading

A lot of women are opting to have their eyebrows and/or the little annoying hairs on their faces removed using threading instead of waxing or shaving. This is the method least likely to damage skin, as it is gentler on skin than tweezing, but it has a major downside: It’s only feasible for use on small areas of the skin as it can be very painful. Waxing, especially when it comes to the eyebrow area: It can cause major skin damage. Waxing removes a top layer of skin every time you use it, Women and men with rosacea and other inflammatory skin disorders, such as psoriasis and eczema should avoid waxing altogether because, although it weakens hair growth, it causes hair growth to actually accelerate during the first three to six months, which increases the need to wax more and can lead to hyperpigmentation.

Laser treatments

We have a winner! Modern lasers are the preferred method, modern lasers can be used to treat an individual’s skin and hair type. After a treatment, hair remains in the skin, but will fall out after four to eight days. And how is this for an incredible added bonus: Depending on the laser used, you may even experience some skin rejuvenation due to the formation of new collagen. A method that removes hair and can fight fine lines.

Large Pores
Typically, large pore size is related to overactive sebaceous gland activity, so those with oily skin or acne tend to experience more visible pores. When these oily skin pores become clogged with sebum, the pore wall becomes stretched. This oily substance, called sebum, produced by our bodies, keeps the skin lubricated and waterproof. It’s common to see patients with naturally oily skin, to have larger pores than patients with dry skin. Our skin starts swelling when oil and dirt collects in our skin, therefore the pores will start to look bigger than what it is. Large pores should not always be associated with severe acne, Sometimes small scars formed after acne, can be wrongly identified as large pores.

Age and “tired” skin function can also contribute to enlarged pores, as can sun exposure and skin that is out of balance due to incorrect product use, health issues, medication use or other reasons.

Treatment Selections

We’ve seen considerable improvements for acne patients with the use of BBL laser and Dermapen .

  • BroadBand light laser, Microlaserpeel and Dermapen – are useful in treatment of enlarged facial pores. This treatment is triple-benefit with skin smoothing, collagen encouragement and pore minimizing.
  • Oral isotretinoin (Accutane) and topical tretinoin (similar to Retin-A)  have shown to be effective in decreasing pore size.
  • Glycolic acid peels, with or without vitamin C, have been shown to decrease pore size appearance. Salicylic and Pumkin acid peels are also helpful for acne patients because this type acid penetrates plugged pores.

At home treatment tips

  • Exfoliate regularly. But don’t use the highly abrasive ground shell formulas – they’ll just tear your skin.
  • If you use cream-style cleansers and notice clogged pores, switch to a water-based foaming cleanser.
  • Try a low level exfoliating moisturizer (eg Retinol which can cause mild flaking).
  • Kaolin clay masks can help keep pores clearer. Steer clear of the highly fragranced products.
  • Pay attention to diet and get adequate fruits, veggies and fiber.
  • Don’t sleep in your makeup.
  • Fabric softeners used for sheets and pillow cases can leave a film on your skin. If you notice more pores or small bumps on the sides of your cheeks, try skipping fabric softener and see if you notice an improvement.
  • Don’t try extractions without knowing how. You can easily push thickened oil deeper into the pore, making matters worse.

Lines and Wrinkles
We all age, that is a fact of life, but the way we age is actually a choice we make. More than half of us look older than we really are because we either engage in behaviors that increase our aging, or we do not actively support a more youthful body through inaction. Knowledge is power, and the more you know about fighting the aging process, the more control you can take toward maintaining a healthier, younger body and mind. Other factors can contribute to wrinkle formation including excess sun exposure and smoking. But whatever the cause, wrinkles are one of the most common aesthetic issues that our patients wish to address. Often, wrinkles are not advanced enough to require a full facelift. That’s where Aesthetic treatments such as Laser, Medical needling and injectables come in. These treatments reduce the appearance of wrinkles for a fraction of the cost of a full facelift.

Blemishes as well as sun spots are perhaps the first give-away sign as far as ageing is concerned especially for lighter skins; without doubt the very first manifestations of ageing.

Furthermore, the skin around the eyes and around the mouth is the very first place to show ageing. Some individuals begin to age as early as in their twenties – this applies to damage to the delicate skin experienced in both males and females .

Today something positive can be done to correct these ageing issues and concerns – we refer to none other than technologically advanced anti-ageing skin and laser treatment.

Our Treatment Options:

Pigmentation
It is very important to protect your skin from the sun especially living in South Africa. Our specialist team of Dermatologists and skincare experts offers therapy to people living in and around Cape Town, who want to correct sun damage and other skin hyperpigmentation revealing a healthier, more youthful glow.

Pigmentation or hyperpigmentation refers to dark spots, patches or general skin darkening. Skin darkening is caused due to external and internal factors like UV rays & hormones, which activates the pigment cells to create more melanin.

There are many factors that contribute to the production of melanin, which may cause pigmentation. Being overexposed to the sun or sun damage plays a big role in the appearance of hyperpigmentation. Antibiotics or other medicine such as contraceptives or hormone treatments are also known to play a role. Acne, inflammation of the skin or picking at the skin may cause pigmentation, but pigmentation in the form of freckles may be genetic and hereditary. Sun exposure and hormonal changes can also trigger melasma, which presents itself as pigmented patches of skin. Most patients get melasma on their cheeks, nose, forehead and upper lip, although it can also show up in other sun-exposed areas, such as the neck, chest and forearms. In some cases, melasma will fade on its own, especially if the trigger is removed- such as when a baby is born and the woman is no longer pregnant. For sun-related melasma, we recommend preventing future UV exposure by wearing sunscreen every day and wearing sun protective clothing. If melasma does not fade on its own, a skin-tone-appropriate peel or BBL treatments together with topical skin lighteners such as Hydroquinone, retinol and Vitamin C is recommended.

How to Prevent Excess Pigmentation?

Even though it may not always be possible to prevent hyperpigmentation, there are a few suggestions on how you might minimize the risk of developing it later in life. Most importantly you should always protect your skin from the sun by using a UVA and B protection cream as well as a SPF moisturiser. Be wary of applying perfume to the neck where it might cause pigmentation and try use a brightening cream in your daily beauty regime.

What are the most common types of pigmentation?

  • Hyperpigmentation: generally due to sun exposure and on areas such as face, hands and arms. They are generally small and darkened patches on the skin.
  • Freckles: Freckles are caused by sun exposure and mostly appears on the face.
  • Sun spots – They are sometimes wrongly referred to as liver spots which might appear on the hands and face. This is caused by prolonged exposure to the sun or sun damage to the skin.
  • Melasma/Chloasma: Melasma – This type of pigmentation is believed to be caused by hormonal changes, commonly developed during pregnancy or even while on the pill. It may appear on the cheeks, forehead and upper lip. It shows up on the skin as dark and irregular shaped areas on the face.
  • PIH (Post Inflammatory Hyperpigmentation): This occurs after injury to the skin. The pigmentation is usually flat and discoloured. We see it often after acne scarring.

Pigmentation Treatment:

Prescription creams may help get rid of pigmentation but may take a while to show results.

Chemical peels involve applying chemicals to the skin to help get rid of the top layer and fades the appearance of mild discolouration.

BroadBand Light -BBL™ treatment is ideal if you want your skin to have a healthy glow. There are a variety of laser treatment option types, and the one best suited to you will depend on your skin and pigmentation type. Your skin might be red after a laser Pigmentation treatment, but will be bright and even in the long run.

Rosacea

What is rosacea?

Rosacea is a common chronic, inflammatory disease of the face. It is characterised by redness and red bumps (papules) on the cheeks and possibly the forehead, cheeks, chin and nose. It also sometimes affects the eyes.

Adults, especially women between the ages of 35 and 50, are mostly affected. However, the condition occurs in both sexes and among people of all ages and skin types (even though there is a slightly higher risk among people with fair skin). The exact cause of rosacea is unknown, but it is believed that several factors may be involved. It has been suggested that environmental and genetic factors may play a role and some researchers believe that blood vessels which expand too easily may be a causative factor.

Treatments for Rosacea ?

Amazing, light based technology works to erase many of the unwanted signs of Rosacea. This effective technology allows light to pass through surface skin cells, destroying unwanted pigmentation and blood-vessels. The end result of this treatment is a noticeable decrease in unwanted signs of Rosacea. Your skin tone will immediately become clearer, and the redness will disappear.

These state of the art laser skin-treatments utilize Broadband Light to destroy the surface, unwanted blood-vessels that accompany Rosacea. Although there is no permanent cure for Rosacea, BroadBand Light BBL ™ is an effective treatment for managing the condition. Patients can go through life with normal, healthy, even-coloured skin.

There are many Rosacea-related skin conditions that can be successfully treated with laser treatments, such as:

  • Red or flushed skin
  • Small broken capillaries
  • Rosacea related acne

How do the Rosacea laser treatments work?

Our Rosacea treatments work by exposing the compromised skin to light which destroys the numerous surface veins that make up Rosacea. The net result is the reduction of surface redness, broken blood vessels, splotchy skin colour, and sun-spots.

How many treatments are needed to manage Rosacea?

On average, 3 to 4 v+BroadBand Light BBL ™ treatments are needed to clear-up current Rosacea.

These treatments are relatively quick, treating the entire face takes about 30 minutes.

Do Rosacea treatments hurt?

Most patients describe the treatment as feeling similar to a quick rubber-band snap against the skin. Although not necessarily a pleasant sensation, the treatment is over so quickly that everyone has an easy time getting through it. For the more sensitive, taking an over-the-counter anti-inflammatory (such as Aleve or Motrin) 30-45 minutes before treatment can help alleviate any discomfort.

What will I look like immediately after a Rosacea treatment?

One of the most satisfying things about these Rosacea laser treatments is that the results are almost immediate. After one week post treatment you will notice a decrease in skin redness, which will continue to improve over the following 14- 28  days.

Are the results permanent?

Unfortunately, these treatments do not permanently cure Rosacea, and instead they are just a way to control and and manage it. Maintenance treatments will be needed, but the frequency will depend largely on how you care for your skin in the meantime. Those with Rosacea should avoid any known triggers, such as sun exposure, spicy food, red wine, and over-heating. In addition to performing the treatments, we are also happy to help advise on a proper at-home regimen, and provide tips and tricks for best managing your Rosacea.

Rough texture
Uneven skin texture is commonly a result of excess dead skin cells that build up on the surface of the skin. This can make areas of the skin feel rough or bumpy to the touch and can also give the skin a dull appearance. As for how it happens, there are a number of potential factors.

In addition to dead skin cell buildup, chronic sun exposure can also play a big role in uneven skin texture and pore-size irregularity. Natural aging of the skin will also make the skin look duller. Factors such as smoking may also contribute, and other skin conditions such as eczema and dry skin can severely affect skin tone and texture.”

those who deal with chronic texture issues will see the best results with laser treatments. But there are a few ways to get a more even skin surface at home.

5 Effective Ways to Improve Uneven Skin Texture, Rough Skin and Fine Lines

Sundamage
It feels good to spend time in the sunshine, but it can take a toll on your skin. Research shows that UV exposure is the reason behind 80% of your skin’s aging!

The consequences of sun exposure are both cosmetic and medical. Cosmetic changes include leathering of the skin, increased wrinkling, loss of facial volume due to other ageing effects and enlarged capillaries which leads to redness and uneven complexion. Medical changes include more frequent development of skin cancer, development of , and the tendency of sun-damaged skin to get more easily irritated and bruised.

The sun gives off ultraviolet (UV) . UVA rays penetrate into the deep layer, the dermis, where your skin’s collagen and elasticity lives. When this layer gets attacked by sun damage, free radicals are released and cause harm to your skin cells’ DNA. The result is saggy skin, wrinkles, hyperpigmentation (dark spots and melasma), and potential risk of skin cancer. The signs of sun damage on the skin become visible from as early as the twenties. With the passing of time, these changes become more evident even if sun exposure is reduced or eliminated.

Sun exposure damages five major parts of the skin. This damage can cumulatively involve the epidermis (actinic keratosis), dermis (solar elastosis), blood vessels (telangiectasia), sebaceous glands (solar comedones), and melanocytes (diffuse or mottled brown spots)

Age or Sun Spots

These unwelcome patches of darkened skin are a result of photodamage. Age Spots are sometimes also referred to as ‘liver spots’ or ‘sun spots’. These spots can occur on the face, hands, the back, legs, arms and décolletage.

Melasma

Melasma is a chronic condition which requires treatment and maintenance. The exact cause of Melasma is uncertain, but typically it develops in women during their reproductive years and is thought to be triggered by factors such as sun exposure, contraceptive medication or hormonal fluctuations during pregnancy. Melasma, also known as hormonal pigmentation, is a skin condition characterized by asymmetrically shaped patches of brown or grey skin discolouration. It affects mostly women, but some men have been known to develop the condition.

Treatment for sun-damaged skin

One of the best things you can do for your skin is to start protecting it today and stop any future damage from occurring.

There are many anti-aging creams on the market. But sometimes advanced signs of aging and sun damage require more invasive dermatology procedures, such as:

  • BBL (Broadband light) therapy is the gold standard when it comes to treating sun-damage. BBL is different from laser therapy because it delivers multiple wavelengths of light with each pulse (lasers deliver only 1 wavelength). BBL is a type of non-ablative therapy. This means it does not damage the top layers of skin.
  • Tretinoin treatment. This prescription skin cream can reduce wrinkles, rough skin, and discoloured skin.
  • Chemical peels. Chemical peels are often used to minimize sun-damaged skin, irregular skin colour (pigment), and superficial scars. The top layer of skin is removed with a chemical applied to the skin. By removing the top layer, the skin regenerates. This often improves its appearance.
  • Laser skin resurfacing. Laser skin resurfacing uses high-energy light to burn away damaged skin. Laser resurfacing may be used to minimize wrinkles and fine scars. It is also used to reduce enlarged oil glands, lighten up sun spots, and remove wart-like growths.
  • Filler injections. A soft tissue filler is injected beneath the skin to replace the body’s natural collagen that has been lost. Fillers are generally used to treat wrinkles, scars, and facial lines.
  • Botulinum toxin type A. Using botulinum will also soften existing wrinkles. But it won’t prevent the signs of aging. It is most often used for forehead lines and the lines between and around the eyes.
